What Is Wegovy?

Wegovy is the brand name for high-dose semaglutide (2.4 mg weekly), manufactured by Novo Nordisk. It was FDA-approved in June 2021 specifically for chronic weight management in adults with a BMI of 30 or higher, or 27 or higher with at least one weight-related health condition. In March 2024, it received an additional approval for reducing cardiovascular risk in adults with obesity and established heart disease.

The STEP Trials: What the Research Shows

Wegovy's approval was supported by the STEP clinical trial program — a series of large, randomized, placebo-controlled trials. The headline result from STEP 1 was an average weight loss of approximately 15% of body weight over 68 weeks in adults without diabetes. Approximately one-third of participants lost 20% or more of their body weight. These results were unprecedented for a pharmacological intervention.

Titration Schedule

Wegovy follows a 16-week titration: 0.25 mg for weeks 1–4, 0.5 mg for weeks 5–8, 1.0 mg for weeks 9–12, 1.7 mg for weeks 13–16, then 2.4 mg maintenance from week 17 onward. This gradual increase is designed to minimize gastrointestinal side effects.

A Mass General Brigham analysis found these private MFN deals could reduce projected Medicare savings by nearly 80% compared to the separate GLOBE/GUARD Medicare pricing models — which were proposed to take effect October 1, 2026 (GLOBE) and January 1, 2027 (GUARD).
